What Your Annual Blood Tests Actually Tell You

Dr. Peter Attia, a physician specializing in longevity medicine, advocates for an expanded annual panel that includes fasting insulin (to detect insulin resistance 5-10 years before glucose elevates), ApoB (a more precise cardiovascular risk marker than LDL-C, per a 2022 JAMA Cardiology meta-analysis of 24 studies, n=330,000), and high-sensitivity CRP. A standard annual blood panel includes a complete blood count (CBC), comprehensive metabolic panel (CMP), and lipid panel. Together, these roughly 30 numbers provide a snapshot of organ function, infection status, and metabolic health. Yet most patients receive their results with little explanation beyond a green checkmark or a red flag.

The complete blood count measures red blood cells, white blood cells, and platelets. Hemoglobin below 12 g/dL in women or 13.5 g/dL in men indicates anemia worth investigating. But mild elevations in white blood cell count (10,000-12,000/mcL) often reflect nothing more than the stress of the blood draw itself or a recent workout. Context matters more than the reference range.

Key finding: The metabolic panel reveals kidney and liver function. Dr. Andrew Levey, professor of medicine at Tufts Medical Center and lead author of the CKD-EPI equation used worldwide, notes that eGFR (estimated glomerular filtration rate) synthesizes creatinine, age, sex, and race into a single kidney health score. Below 60 mL/min sustained over three months defines chronic kidney disease — but a 2021 NEJM update by Levey's group removed the race variable, improving accuracy for Black patients.

Liver enzymes (ALT, AST) are among the most frequently misinterpreted values. Dr. Paul Watkins, professor of medicine at UNC Chapel Hill and director of the Hamner-UNC Institute for Drug Safety Sciences, has shown that mild elevations (under 2x the upper limit) can result from intense exercise, alcohol consumption within 48 hours, acetaminophen use, or even celiac disease. A Gastroenterology 2017 guideline from the American Gastroenterological Association recommends repeating mildly elevated values before pursuing imaging or biopsy.

The lipid panel has evolved beyond total cholesterol. Dr. Allan Sniderman, professor of cardiology at McGill University, published a 2021 JAMA Cardiology meta-analysis (k=24, n=330,000) demonstrating that ApoB — a direct measure of atherogenic particle count — outperforms LDL-C as a predictor of cardiovascular events, particularly in patients with normal LDL but elevated triglycerides. LDL-C remains the primary treatment target, but ApoB provides additional resolution when LDL-C and cardiovascular risk appear discordant.

Fasting glucose and HbA1c together assess blood sugar regulation. A fasting glucose under 100 mg/dL is considered normal, but Dr. Elizabeth Selvin, professor of epidemiology at Johns Hopkins, demonstrated in a Lancet 2010 study (n=11,092) that HbA1c between 5.7% and 6.4% identifies prediabetes even when fasting glucose looks fine. The two tests measure different windows: fasting glucose reflects this morning, HbA1c reflects the past 90 days.
