Caffeine and Health: The Full Risk-Benefit Analysis

Caffeine is consumed daily by approximately 80% of American adults, making it the most widely used psychoactive substance on the planet. The FDA considers up to 400 mg daily (roughly four 8-oz cups of brewed coffee) safe for healthy adults. Individual response varies dramatically: Dr. Ahmed El-Sohemy (University of Toronto) identified the CYP1A2 gene variant that divides the population into fast and slow metabolizers — slow metabolizers face elevated cardiovascular risk at the same doses that appear protective in fast metabolizers.

The benefits are well-documented. A 2024 BMJ umbrella review (k=201 meta-analyses, led by Dr. Robin Poole at the University of Southampton) confirmed that 3-5 cups of coffee daily is associated with a 25-30% reduction in type 2 diabetes risk, a 29% reduction in Parkinson disease, and significantly lower rates of liver cirrhosis and hepatocellular carcinoma. Decaffeinated coffee retains the metabolic and liver benefits, suggesting polyphenols and chlorogenic acids contribute independently of caffeine.

Key finding: A 2013 Journal of Clinical Sleep Medicine study by Dr. Christopher Drake (Henry Ford Hospital, n=12) demonstrated that caffeine consumed six hours before bedtime still reduced total sleep by 41 minutes on polysomnography — even when participants reported no subjective difficulty falling asleep.

Sleep disruption is the most significant adverse effect. Caffeine has a half-life of 5-6 hours, but in CYP1A2 slow metabolizers it can extend to 9-11 hours. Anxiety and caffeine form a dose-dependent relationship. Above 400 mg, anxiogenic effects become common. The exceptions: anxiety disorders, sleep difficulties, pregnancy, certain cardiac arrhythmias, and interactions with fluvoxamine, ciprofloxacin, and other CYP1A2 inhibitors that dramatically slow caffeine clearance.
